Everything else on the form is already filled in. Do not change anything not listed here, especially the applicant's details on pages 4 and 5, and leave page 6 completely blank.

Do this first. Go to i94.cbp.dhs.gov → Get Most Recent I-94 → read the Admit Until date. Use the I-94, not your visa page, they can differ. If that date is after the applicant's departure, carry on. If it is before, tell me and stop here.

Then send four things

  1. Two or three recent payslips, or an employer letter with your title and salary. Must support item 5 above.
  2. A bank statement for each account you listed at item 6, showing that balance.
  3. Your I-94, downloaded from i94.cbp.dhs.gov.
  4. A photo of your visa page, and your approval notice.

You do not need to be wealthy. Any genuine, documented amount is fine. What matters is that your permission to be in the US covers the whole visit. The financial figures are signed under penalty of perjury so they must be true, but the form is not enforceable against you the way a green card sponsorship is, and nobody can pursue you for the applicant's costs.
